There are so many neutral endings that break my heart but this one struck me the most. Not only does it start with Sans not doing his usual “i’ll leave a message” but it’s entirely Alphys being honest with her own emotions and taking up responsibility. In most every other ending it’s implied that the stress was too much or that she’s fruitlessly working with Undyne, but in this she’s become the leader with Sans hanging around with her.
And I don’t know if it’s just me or what but I feel there’s a deep echo in that resentment of “i should’ve done it when i had the chance” between these two survivors.
It also just adds more fuel to the theories that these two used to work together in the past, until something happened to make Sans and Papyrus move back to Snowdin together. Alphys recognizes Sans in the True Ending and is surprised to see him again, probably even glad?
And that start just makes me think of Sans sitting with an anxious Alphys in the lab tracking down your number, because in this route you never befriended Papyrus, never got on his phone, and Toriel is just dust behind a locked door.

Gosh, so heartwrenching. Even though Alphys became stronger, the cost is simply to tragic to bear. 
To me, the neutral ending that got my heart the worst is when you befriend Undyne, but betray her by killing a monster afterwards. And the heartache is all because of what Papyrus said for you to do...
